On-site Registration for
Fall 2006 Season is complete
If this is your child's first season playing with WYSA,
you must provide a copy of his/her birth certificate.
Please remember that the player must be 4 years old by August 1, 2006 to play Fall 2006.
Age Groups and Fees (not including shorts, shin-guards, shoes or ball)
U5 and U6 (mixed boys and girls) - $65 (includes shirt
and socks)
U7 boys, U7 girls, U8 boys, U8 girls, U10 boys and U10 girls - $70 (includes shirt and socks)
U11-12, U13-14 and
U15-16 Rec - $85 (includes jersey and socks)
Note: If more than three (3) children in a family
play recreational or recreational-plus (not travel) soccer, registration for the fourth or more child is free.
Late Player Registration Procedure ($25 late fee if
mailed after July 24, 2006)
Coach Registration Procedure

(print, fill in and mail to POBox 346, Westfield IN 46074). REMEMBER TO ADD THE $25 LATE FEE TO YOUR CHECK
IF YOU SEND IT IN AFTER MONDAY JULY 24, 2006. MAKE CHECK PAYABLE TO "WYSA". Late player registration does not necessarily
guarantee a place to play.
Coaches must complete the online Risk Management Form if they did
not do so for the Fall 2005 or Spring 2006 season. Here is the link (you must copy and paste the link into your browser, clicking
